Apple has introduced hypertension notifications in watchOS 26 to help users stay informed about potential patterns of high blood pressure. This feature adds another layer to Apple Watch’s role as a health companion, alongside irregular rhythm notifications, low heart rate alerts, and ECG. While it does not measure blood pressure directly, it uses advanced algorithms and long-term data analysis to identify signs of hypertension risk.
The system works by monitoring signals from the optical heart sensor and activity data. Over a rolling 30-day period, the watch analyzes your trends and, if a pattern consistent with hypertension appears, it will send you a notification. This prompt encourages you to confirm the findings with a clinically validated blood pressure monitor and share results with a healthcare professional. Apple emphasizes that this feature is not diagnostic and should be treated as an early warning system rather than a replacement for medical equipment.
To enable hypertension notifications, you must be running the latest iOS 26 on a compatible iPhone and watchOS 26 on a supported Apple Watch. Once those requirements are met, you can set up the feature directly in the Health app on iPhone.
- Open the Health app on your iPhone.
- Tap your profile picture.
- Select Health Checklist under Features.
- Find Hypertension Notifications and follow the prompts to turn it on.
- Confirm your details and complete setup.
After activation, the feature runs quietly in the background. If Apple Watch detects a potential hypertension pattern, you’ll receive an alert with guidance. You’ll also be prompted to log a week of blood pressure readings using a proper cuff to verify the trend. These logs provide actionable data for your doctor and help confirm whether elevated blood pressure is present.
Hypertension notifications are available in 159 countries and regions. If the option does not appear in your Health app, it is likely due to regional restrictions or device compatibility.
Supported devices
Hypertension notifications require:
- iPhone 11 or later with iOS 26
- Apple Watch Series 9 or later or Apple Watch Ultra 2 or later
- The latest version of watchOS 26
- Wrist Detection enabled in Apple Watch settings
The feature is not supported on Apple Watch SE or older Apple Watch models. Users must also be 22 years or older and not pregnant in order to turn it on.
Hypertension notifications add to Apple Watch’s growing list of proactive health tools. By combining heart data and machine learning, Apple provides users with early insights into possible risks. While it cannot replace traditional monitoring, the feature gives you a reason to measure more consistently and to act sooner by consulting a healthcare professional.
