Apple could be planning its biggest Apple Watch Ultra update since the product first launched in 2022. Multiple new reports claim the upcoming Apple Watch Ultra 4 will feature a major redesign alongside upgraded health sensors and new blood pressure monitoring capabilities.

The Apple Watch Ultra lineup has mostly seen incremental updates over the past few years. The second-generation model added a brighter display, while the Ultra 3 focused on internal improvements like 5G support. If the latest supply chain reports are accurate, Apple is finally preparing a much larger overhaul for the Ultra series in fall 2026.
One of the biggest rumored additions is a new blood pressure monitoring feature. According to DigiTimes, Apple is developing a system that uses the optical heart-rate sensor to analyze how blood vessels react during each heartbeat. The feature could detect abnormal patterns linked to hypertension and send alerts to users. Reports say the technology is currently under FDA review.
Apple already introduced Hypertension Notifications in watchOS 26, but the new system appears to be a more advanced implementation with improved sensing accuracy and clinical validation. It is still unclear whether the Apple Watch Ultra 4 will provide exact blood pressure readings or simply warn users about potential high blood pressure trends.
The redesign itself could also be substantial. Supply chain sources claim Apple may double the number of sensors on the back of the watch, potentially moving to a new ring-style sensor layout. Reports describe the upcoming model as a “full redesign” with major upgrades to sensing functions and overall hardware efficiency.
The Apple Watch Ultra has kept the same rugged titanium design language since launch, so a visual refresh would mark the first major exterior redesign for the lineup. While exact changes remain unknown, Apple could reduce the size of the sensor housing, refine the casing, or improve battery efficiency through updated internal components.
The health upgrades reportedly extend beyond blood pressure tracking. Apple is still actively working on noninvasive blood glucose monitoring technology, though that feature is not expected to be ready for the Apple Watch Ultra 4. Current reports suggest glucose monitoring may arrive in future Apple Watch models after additional regulatory approvals.
2026 is also shaping up to be a major year for Apple’s Ultra branding strategy. Rumors point to Apple launching an iPhone Ultra foldable, a MacBook Ultra, and new AirPods with cameras alongside the Apple Watch Ultra 4. Apple appears to be positioning the Ultra lineup as its most premium hardware category across multiple product segments.
The Apple Watch Ultra 4 is expected to launch alongside the iPhone 18 Pro lineup and Apple Watch Series 12 during Apple’s usual September event window later this year.
